Sleep gets disrupted due to Apnea, as the breathing passage closes and body organs have to apply increased pressure to open it up. It can happen multiple times during one night. CPAP is the most effective remedy for this but it is not very convenient and it is costly also. There are three types of Apneas. The most common type is Obstructive Sleep Apnea (OSA). For mild to moderate OSA disorder, oral device called Mandibular Advancement Device (MAD) can be used very effectively. It looks more like a mouth guard. Sleep Apnea is a serious disorder where throat walls close completely thus obstructing breathing. It is a very common disorder and can occur in persons of all ages including babies. People with Apnea stop breathing during sleep. During an apnea episode, the diaphragm and chest muscles have to work harder as increased pressure is required to open the airway. Breathing usually resumes with a loud gasp or body jerk. These episodes interfere with sleep, reduce the flow of oxygen to vital body organs, and may cause heart rhythm problems.

 

Apnea is of three types: Obstructive sleep apnea (OSA), Central sleep apnea, Complex sleep apnea syndrome. There are a number of treatments like CPAP, Surgery for Apnea while oral devices can be used effectively for mild to moderate Obstructive sleep apnea. Mandibular advancement device (MAD) is a mouth guard used to treat Obstructive Sleep Apnea; another oral device stabilizes the tongue.

 

Who Should use MAD ?

Persons with mild to moderate obstructive sleep apnea, young persons, who sleep on their side, and those with receding jaw structure. Your doctor will examine and decide accordingly.

How MAD works

It moves the lower jaw forward thus widening the breathing passage. The plates are adjustable allowing advancement level of your jaw by millimetres every increment. The mechanism to advance varies for each device.

 

Types of Devices

Tongue retaining device is less commonly used than MAD and it is splint that holds the tongue in place to keep the airway open.

 

MAD is of two types

Boil & Byte: made of thermoplastic, available over the counter. It is boiled to soften it up and placed in mouth. The hard bite gives it the required shape.

Custom made: Dentist will take the impression and the lab will create the required plates based on that.  

MAD is a more convenient device than CPAP and does not cost as much. It requires no electricity and can be worn on the move.

 

Risks

The oral appliance MAD can cause bite changes, possibility of TMJ disorder, pain in the jaw or teeth becoming loose, excessive drooling and dryness in the mouth.
